Our Social Non-physician Home Health Case Management Unit Offers Advance Care Planning (ACP)
Our Advance Care Planning (ACP) team offers a face-to-face assessment between our professional and qualified health coordinators/agents, and our patients and/or their family members, to develop a Health, Resource, and Estate plan. This is to determine a patient’s health care wishes if they should become unable to make decisions about their overall care.
While this service is covered by Medicare Part A through a Medical Wellness Visit (MWV), or Part B through a cost sharing model, Avion Group offers an uninsured program model to ALL patients who are not covered. This is extended through our nonprofit IRS Recognized 501( c )(3) Social Health Care Program, which includes a “No Questions Asked” element to all immigrants who have reservations about their health care management because of their resident status in the United States. These services can be performed at our office, facility, the patient’s home, an SNF, and at times through our telehealth unit.
In addition to Healthcare and Estate Planning components covered within this service, our economic health coordination unit supplies patients with all the resources required that parallel to their COVID-19 diagnosis to assist them socially, medically, and economically, as to correlate these components to create health services that strategize the overall patients care plan. These services are conducted under the supervision of a team that may include a variety of healthcare, estate, and case management professionals.
